The IVF process

IVF step by step
Five phases, four to six weeks.

Short answer

IVF is a 4 to 6 week process in 5 phases: ovarian stimulation, egg retrieval, fertilization and culture in the lab, embryo transfer, and the two-week wait before the pregnancy test. Each phase has a specific goal, and knowing which one you are in reduces anxiety.

Phases 1 and 2

Stimulation and retrieval

Phase 1: ovarian stimulation

In a natural cycle only 1 egg matures. Here the goal is to get multiple eggs.

  • Daily subcutaneous injections of gonadotropins (FSH ± LH), usually at night, in the abdomen or thigh
  • Ultrasound every 2 to 3 days to track follicle growth, and estradiol levels to fine-tune the doses
  • When the follicles reach the ideal size (18 to 20 mm), a “trigger” shot (hCG or a GnRH agonist) for final maturation
  • Egg retrieval exactly 34 to 36 hours after the trigger

Most women describe the injections as “less painful than I expected.”

Phase 2: egg retrieval

An outpatient procedure under light sedation (not general anesthesia), after 8 hours of fasting. Guided by transvaginal ultrasound, a thin needle collects the fluid from each follicle, and the lab identifies the eggs. You are usually discharged within 2 hours, with rest recommended for the next 24 to 48 hours.

Phases 3 to 5

From the lab to the test

Phase 3: fertilization and culture

  • Conventional IVF places eggs and sperm together in a culture dish; in ICSI, one sperm is injected directly into each mature egg
  • At 16 to 20 hours, fertilization is confirmed
  • Day 3: embryos at the cleavage stage (4 to 8 cells)
  • Day 5 to 6: blastocysts, a more advanced stage, better for transfer
  • Embryos can be transferred fresh or frozen for future use

Phase 4: embryo transfer

It can happen in the same cycle (fresh transfer) or a later one (frozen); many centers now prioritize frozen transfer for slightly better rates. No anesthesia, 10 to 15 minutes, with a moderately full bladder: a thin catheter places the embryo(s) in the uterus under abdominal ultrasound. After 15 to 30 minutes of rest you go back to normal activities (there's no evidence that prolonged rest improves outcomes), and progesterone supplementation begins, usually vaginal.

Phase 5: the two-week wait

  1. Day 9 to 11 after the transfer: implantation happens, if it works
  2. Day 12 to 14: blood beta-hCG test, more sensitive than a drugstore test
  3. Positive: progesterone continues, with an ultrasound in 2 weeks
  4. Negative: the medication stops, the cycle ends, and planning begins for the next attempt

This tends to be the most emotionally difficult stretch of the whole process.
