The standard dose is 400 to 800 mcg a day, started at least 3 months before conception and kept up through the first trimester. Taken that way, folic acid reduces the risk of neural tube defects by up to 70%. Some women need a much higher dose of 4 to 5 mg a day.
Folic acid prevents neural tube defects such as spina bifida and anencephaly. The neural tube, which will become the brain and spinal cord, is already being formed in week 5 of pregnancy and keeps forming through the first 12 weeks. That's why the book calls folic acid the absolute priority of preconception and the most important nutrient for preventing these malformations.
The protection, up to 70% less risk, depends on timing: it applies when folic acid is started at least 3 months before conception and maintained through the first trimester.
| Situation | Daily dose |

| Standard, while trying | 400 to 800 mcg |
| History of a child with a neural tube defect, diabetes, or taking anticonvulsants | 4 to 5 mg |
| After a positive test, through week 12 | 800 to 1,000 mcg |
| Your partner | 400 mcg |

Form: folic acid or methylfolate. Methylfolate is indicated for those with the MTHFR genetic variant.
Yes, the dose for him is on purpose: folic acid is part of the targeted supplements for men with borderline sperm quality.
“What's the ideal folic acid dosage for me?” is one of the questions on the book's preconception visit script. The B12 and folic acid test is also one of the 11 preconception tests, so your doctor can adjust the dose to your history and results.
In other words, folic acid is not something to add once you're pregnant. The up to 70% protection depends on starting before conception, which is why the book makes it the first supplement of Station I.
Folic acid opens Chapter 08 of Map to the Positive, on smart supplementation, in Station I, Foundation. The map treats it as step one, not the whole plan: the same station covers the 11 preconception tests, vitamin D and iodine, and the supplements that only make sense in specific situations, and Station V picks folic acid back up in the first weeks after the positive.
