With the pill, fertility comes back quickly: immediately with the progesterone pill and immediately after the cycle with the combined pill. What takes longer is regularity: cycles usually need 1 to 3 months to settle after the combined pill and 1 to 2 months after the progesterone pill.
Stopping the pill looks simple, but the way you do it affects how long your cycles take to return to normal, and your peace of mind in the first months of trying. It helps to separate two things: when your fertility comes back, and when your cycle becomes regular again. With the pill, the first happens well before the second.
| Method | Cycle return | Fertility return |
|---|---|---|
| Combined pill | 1 to 3 months | Immediate after the cycle |
| Progesterone pill | 1 to 2 months | Immediate |
Average return times by method, from Chapter 9 of Map to the Positive.
These are averages, and each method has its own return time. If you used a ring, patch, IUD, implant or injection, the numbers are different. The book's question script for the preconception visit includes this exact point: How should I come off my birth control? How long to wait? Take it to your doctor.
In the first months off the pill, cycles may come back irregular, the flow may change in intensity, and PMS symptoms may resurface. They were there before: the external hormone was masking them. This is normal and usually temporary.
To know when regularity is back, compare your cycles with what the book defines as a normal cycle:
If regularity hasn't returned after 3 cycles, it's worth talking to your doctor. It may just be the natural time of adaptation, or there may be an underlying condition (PCOS, for example) that the pill was masking.
These can be signs of PCOS, hypothyroidism or other conditions that were masked by the external hormone. The sooner you investigate, the better.
Coming off birth control is Chapter 9 of Map to the Positive, in Station I (Foundation). The same station gives you a 20-minute self-assessment, the 11 preconception tests, smart supplementation with folic acid as the priority, the real detox, vaccines and dental care, and a question script for the preconception visit. Then Station II teaches you to read your cycle as it comes back: the fertile window, cervical mucus, basal temperature and ovulation tests.
