Yes. The old advice that “you have to wait 3 cycles” is outdated: studies show no increased risk of miscarriage or malformations in pregnancies that happen in the first cycle after birth control. The one condition is folic acid, which should have been taken for at least 1 month, ideally 3.
Many women still hear that they must let a few cycles go by before trying. The book is direct about it: you don't have to wait to try. Pregnancies that start in the first cycle after birth control show no increased risk of miscarriage or malformations.
Myth: after stopping birth control, wait 3 cycles before trying.
Fact: the first cycle already counts. What needs a head start is folic acid.
If you plan to try from the first cycle, folic acid has to start before you stop birth control. It is the best-studied supplement of preconception: it reduces the risk of neural tube defects (spina bifida, anencephaly) by up to 70% when started at least 3 months before conception and maintained through the first trimester.
The book's preconception question script includes What's the ideal folic acid dosage for me? Ask it before you stop your method.
Whether the first cycle can count depends on how fast your method lets fertility return. The book's averages:
| Method | Fertility return |
|---|---|
| Progesterone pill | Immediate |
| Copper IUD | Immediate |
| Hormonal IUD | Usually immediate |
| Combined pill, ring, patch | Immediate after the cycle |
| Implant | Within 1 month |
| Monthly injection | 3 to 6 months |
| Quarterly injection | 6 to 18 months |
Fertility return by method, from Chapter 9 of Map to the Positive.
Trying in the first cycle is safe, but it is one cycle. Even for a healthy woman with well-timed intercourse, the chance of pregnancy in a single cycle is roughly 20% to 25%. If you are four cycles in with no positive, you are still perfectly within the curve.
The first cycles after birth control can also come back irregular, which moves the fertile window. When cycles are irregular, reading your body directly (cervical mucus, temperature, secondary signs) is more reliable than any calendar calculation.
Trying in the first cycle comes from Chapter 9 of Map to the Positive, in Station I (Foundation), together with the return time of each method and the signs that deserve a doctor's attention after stopping. Station I also covers smart supplementation, the 11 preconception tests and the preconception visit. Station II then teaches you to find the fertile window in your own body, so that first cycle is used well.
